MACS Equipment Loan Agreement Between MACS and MACS’s Families for Use of MACS Chromebook
Purpose:
This document outlines an agreement regarding the use and distribution of MACS technology equipment (i.e. Chromebook, battery pack, charger, etc.). All MACS users of MACS assigned technology equipment share in these responsibilities.
Families being assigned this equipment shall sign this form as a method of record of the assignment and acknowledgement of the responsibility of caring for the technology equipment properly for the duration of the loan. The parent/guardian signing also acknowledges instruction in use and care of technology equipment.
Equipment Use and Care Guidelines:
● Proper care and security of the technology equipment is the responsibility of the parent/guardian and student to whom it is assigned for the duration of the assignment.
● Technology equipment will be assigned to a parent/guardian and student for the length of the assignment. At the conclusion of the assignment, the technology equipment must be returned to the Principal or his/her designee.
● MACS technology equipment is intended as a resource for educational purposes during the course of a family’s responsibilities to the assignment.
● MACS is not obligated to purchase additional software or peripherals beyond the identified standard for the school.
● Throughout the assignment of the technology equipment, it may be required to be returned for short periods to technology staff for servicing and/or upgrading.
● All maintenance on the technology equipment will be performed through technology staff.
● In the case of technology equipment failures, please notify technology staff in writing via the Home Chromebook Damage Report ( https://www.macs.k12.pa.us/apps/form/HomeCBDamageRpt ). A technology staff member will work with the family to facilitate the repair or replacement of the equipment.
● If the technology equipment failure is a result of abuse or neglect, the family to whom the technology equipment is assigned may be assessed the cost for the repair or replacement of the equipment.
● If the student is found liable for damages and payment is not received, security deposits may be forfeited.
● Theft or loss of the technology equipment must be reported to the Principal. An incident report must be generated. In the case of theft, a police report must be filed within 24 hours.
● All comments/notes on the condition of the technology equipment when loaned will be listed on this form.
