MACS Equipment Use Agreement Between MACS and MACS’s Families for Use of MACS Technology

 

Purpose:

Students will be utilizing multiple forms of technology during the school day.  This document outlines an agreement regarding the use and distribution of all MACS technology equipment (including, but not limited to Chromebooks, battery pack, charger, etc.).  All MACS users of MACS assigned technology equipment share in these responsibilities. Families and Students being assigned this equipment must sign this loan agreement to acknowledge and ensure the responsibility of properly caring for the technology equipment for the duration of the loan.  The parent/guardian signing also acknowledges review of the use and care of technology equipment identified below and assumes all risk.


Equipment Use and Care Guidelines:

 

  • Proper care and security of the technology equipment is the responsibility of the parent/guardian and student to whom it is assigned for the duration of the loan.
  • Technology equipment will be assigned to a parent/guardian and student for the length of the loan.  At the conclusion of the loan, the technology equipment must be returned to the Principal or his/her designee.
  • MACS technology equipment is intended as a resource for educational purposes during the course of a family’s responsibilities of the loan.
  • MACS is not obligated to purchase additional software or peripherals beyond the identified standard for the school.
  • Throughout the loan of the technology equipment, it may be required to be returned for short periods to technology staff for servicing and/or upgrading.
  • All maintenance on the technology equipment will be performed through technology staff.
  • In the case of technology equipment failures or damages, students are to notify their teacher and technology staff immediately.
  • A technology staff member will work to facilitate repair/replacement of the equipment.
  • If the technology equipment failure is a result of abuse, neglect, or failure to report by the student, the family to whom the technology equipment is assigned may be assessed the cost for the repair or replacement of the equipment.
  • If the student is found liable for damages and payment is not received, security deposits may be forfeited.
  • Theft or loss of the technology equipment must be reported to the Principal.  An incident report must be generated.  In the case of theft, a police report must be filed within 24 hours.
