| 1) The Application of Intent is now available for the 2013-2014 school year. You may download the Application of Intent from our website (www.macs.k12.pa.us) or obtain one in-person at 3821 N. Broad Street (Broad and Butler Streets). Click HERE for Application of Intent. |
| 2) Once you have completed the Application of Intent and turned it in to our main office, you will be invited to an Information Session in the spring of 2013, that both the parent/guardian and student must attend in order to enter the lottery and/or to enroll at MACS. Please note: Late arrivals will not be admitted to the Information Sessions. |
| 3) Lottery and Enrollment: At the end of the Information Session, if you would like to enroll your child at MACS, you must fill out a Lottery Application Form. Your child's name will then be entered into the lottery for admission (exact date of lottery TBD). |
| 4) If your child's number is selected, you will be contacted by mail and will be provided with information on how to accept admission and enroll. Enrollment will be subject to the timely submission of the following items: a) Proof of Address (utility bill, lease, mortgage statement, or driver's license) b) Student's Final Report Card from prior school c) Anyone of the following documents: i. Health Insurance Card ii. Legal Document iii. Tax Documents (federal income) d) Birth Certificate or i. Green Card ii. Passport iii. Baptismal Papers iv. Receipt - Applied for certificate e) Legal Documents: Foster Parent, Legal Guardian, Adoption papers, etc., if applicable f) Immunization Records (shots and vacines) |
| 5) We also request the following documents, however, they are not required: a) Standardized Test Results b) IEP, ER, NOREP (Special Education) c) Physical exam within the last 12 months |
| 6) The deadline for submitting all of the above items necessary for enrollment and rostering is June 30. Once all of the above items are received by MACS, enrollment will be complete, your child will be rostered, and a letter of confirmation will be mailed to you. However, if the June 30 deadline is not met, the student will lose any and all rights to a seat gained through the lottery system, will not be enrolled, and will be placed at the end of the waiting list, pending any no-shows of rostered students after the start of the school year |
